1. New Patients, please arrive 15 minutes before your appointment time and bring your insurance card and a list of your current medications
2. Please fill out Medical History Form (click)
3. Please fill out Registration Form (click)
4. All patients under 18 years of age MUST be seen with a parent or legal guardian present at each visit
5. If you need to cancel or reschedule an appointment, we kindly ask that you give us 24 hours advance notice.
